Subkliinisen hypotyreoosin hoidon vaikuttavuus

Tausta:

Subkliinisessä hypotyreoosissa TSH on pysyvästi yli viitealueen ylärajan ja T4-V normaali. Subkliininen hypotyreoosi on nykykäsityksen mukaan oireeton tai erittäin vähäoireinen tila. Näytönastekatsauksessa on huomioitu 8 satunnaistettua ja kontrolloitua tutkimusta «Kong WM, Sheikh MH, Lumb PJ ym. Kaikissa tutkimuksissa verrattiin T4-hormonia lumehoitoon subkliinisen hypotyreoosin hoidossa. Tutkimuksia, joissa olisi verrattu T3-hormonia, eläinperäisiä kilpirauhasvalmisteita, jodia tai seleeniä lumehoitoon subkliinisen hypotyreoosin hoidossa, ei löydetty. Tutkimushenkilöt olivat iältään 18–65-vuotiaita viidessä tutkimuksessa «Kong WM, Sheikh MH, Lumb PJ ym. Tutkimusten kesto oli 3–12 kuukautta. Suurin osa tutkimuksista oli laadullisesti heikkoja, lyhytkestoisia ja pieniä. Ainoastaan yksi tutkimus oli iso ja hyvin laadukas. Kaikki siihen osallistuneet henkilöt olivat yli 65-vuotiaita «Stott DJ, Rodondi N, Kearney PM ym. Thyroid Hormone Therapy for Older Adults with Subclinical Hypothyroidism. N Engl J Med 2017;376:2534-2544 »6. Yhdessä tutkimuksessa kaikki osallistujat olivat vähintään 80-vuotiaita «Mooijaart SP, Du Puy RS, Stott DJ ym. Association Between Levothyroxine Treatment and Thyroid-Related Symptoms Among Adults Aged 80 Years and Older With Subclinical Hypothyroidism. JAMA 2019;322:1977-»8. Terveystaloudellista näkökulmaa selvittäviä tutkimuksia ei löydetty.

Tulokset:

Tulokset olivat vaihtelevia. Kun kaikkia tutkimuksia tarkasteltiin yhdessä, hypotyreoosiin liittyvässä elämänlaadussa, yleisessä elämänlaadussa, masennusoireissa tai hypotyreoosioireissa ei havaittu kliinisesti merkitsevää eroa T4- ja lumeryhmien välillä. Yhdessä tutkimuksessa T4-ryhmässä havaittiin hieman heikentynyttä elämänlaatua mielialaan liittyen «Kong WM, Sheikh MH, Lumb PJ ym. A 6-month randomized trial of thyroxine treatment in women with mild subclinical hypothyroidism. Am J Med 2002;112:348-54 »1, ja toisessa tutkimuksessa havaittiin T4-ryhmässä hieman parempaa henkistä hyvinvointia muutamissa osatekijöissä «Reuters VS, Almeida Cde P, Teixeira Pde F ym. Effects of subclinical hypothyroidism treatment on psychiatric symptoms, muscular complaints, and quality of life. Arq Bras Endocrinol Metabol 2012;56:128»5. Koettu vointi koheni useassa tutkimuksessa myös lumeryhmässä. Puuttuva ero oireiden ja elämänlaadun osalta selittynee sillä, että subkliininen hypotyreoosi yleensä on oireeton tai hyvin vähäoireinen tila. Tyroksiinihoito ei parantanut yli 65-vuotiaiden subkliinistä hypotyreoosia potevien henkilöiden kognitiota lumehoitoon verrattuna «Parle J, Roberts L, Wilson S ym. A randomized controlled trial of the effect of thyroxine replacement on cognitive function in community-living elderly subjects with subclinical hypothyroidism: the Bi»7.

Kommentti: Aihepiiristä löytyy vain yksi laadukas ja iso kontrolloitu tutkimus «Stott DJ, Rodondi N, Kearney PM ym. Thyroid Hormone Therapy for Older Adults with Subclinical Hypothyroidism. N Engl J Med 2017;376:2534-2544 »6 ja muut tutkimukset ovat pieniä. Laadukkain, yli 65-vuotiaita koskeva tutkimus on tehty Euroopassa. Pienemmätkin tutkimukset on tehty Suomen olosuhteita hyvin vastaavissa olosuhteissa. Tutkimukset on julkaistu arvostetuissa endokrinologisissa julkaisusarjoissa. Ei löydy näyttöä siitä, että T4-hoito vaikuttaisi oireisiin, mielialaan, elämänlaatuun tai kognitioon subkliinistä hypotyreoosia potevilla henkilöillä.
