- Yli 80 %:lla aivoinfarktipotilaista verenpaine kohoaa reaktiivisesti akuuttivaiheessa normalisoituen jälleen viikon kuluessa «Yatsu FM, Zivin J. Hypertension in acute ischemic strokes. - Kohonnutta verenpainetta suositellaan alennettavaksi heti aivoinfarktin jälkeen ensimmäisen vuorokauden aikana vain erityisestä syystä:
- trombolyysin tai antikoagulaatiohoidonyhteydessä tasolle < 185/110 «European Stroke Organisation (ESO) Executive Committee, ESO Writing Committee. - sydäninfarktin, sydänlihasiskemian tai sydämen vajaatoiminnan yhteydessä
- aortan dissektoitumassa
- hypertensiivisessä enkefalopatiassa

- Useassa observationaalisessa tutkimuksessa paras ennuste on todettu niillä aivoinfarktipotilailla joiden systolinen verenpaine on ensimmäisen vuorokauden aikana ollut välillä 140–180 mmHg, joten optimaalisesta verenpainetasosta aivoinfarktin akuuttivaiheessa ei toistaiseksi voida antaa suositusta «Chamorro A, Vila N, Ascaso C ym. - Verenpaineen alentaminen etenkin vasodilatoivilla lääkkeillä, esimerkiksi kalsiumkanavan salpaajalla, voi heikentää aivojen perfuusiota, joiden niiden käyttöä ei suositella aivoinfarktin akuuttivaiheessa «Grossman E, Messerli FH, Grodzicki T ym. - Akuuttihoidossa eniten käytettyjä antihypertensiivisiä lääkkeitä ovat labetaloli ja enalapriili.
- ACE:n estäjien turvallisuudesta aivoverenkierron kannalta on jonkin verran näyttöä myös akuuttivaiheessa vallitsevan autoregulaation häiriön sekä kaulavaltimoahtaumien yhteydessä «Grossman E, Messerli FH, Grodzicki T ym. - Jos verenpainetta alennetaan, tavoitetaso määritellään yksilöllisesti, yleensä noin 150–180/80–100 mmHg alentamatta keskivaltimopainetta enempää kuin 15–20 % «European Stroke Organisation (ESO) Executive Committee, ESO Writing Committee. - Verenpaineen laskeminen 10–25 % ensimmäisen vuorokauden aikana 48 tunnin kuluessa tehdyn satunnaistamisen jälkeen tavoitellen verenpainetasoa alle 140/90 mmHg 7 vuorokauden kuluessa verrattuna verenpainelääkityksen keskeyttämiseen sairaalajakson ajaksi ei vaikuttanut potilaiden ennusteeseen 14 vuorokauden eikä 3 kuukauden seurannassa mRS-pisteillä mitaten. Kohonneen verenpaineen hoidon aloittaminen akuutissa vaiheessa näyttää turvalliselta ja voidaan toteuttaa yksilöllisesti kliinisen harkinnan mukaan «He J, Zhang Y, Xu T ym. Essentiellin hypertension tavanomaisia hoitosuosituksia voidaan turvallisesti noudattaa aloittaen hoito 3 vuorokauden sisällä oireiden alusta «Lee M, Ovbiagele B, Hong KS ym. - Normaalin verenpaineen kohottamisesta vasopressoreilla ei ole näyttöä ennustetta parantavana hoitona, mutta matalan verenpaineen (systolinen verenpaine < 100 mmHG) hoitaminen on järkevää «Jauch EC, Saver JL, Adams HP Jr ym.
